RUNNING TITLE: Discontinuation of hypnotics in therapy of insomnia
ثبت نشده
چکیده
Many patients suffering of chronic insomnia are prescribed, particularly if treated by nonpsychiatrists, various types of hypnotics. Among these drugs, 3 rd generation hypnotics became increasingly popular. In contrast to benzodiazepines, it is believed that zolpidem may restore physiological sleep 1 .According to Voderholzer et al 2 , zolpidem has only slight and non significant rebound effect and therefore it does not trigger chronic use or a risk of tolerance and dependence. However, there is a group of patients chronically using the drug every day in the long term. Many of them increase the daily dose, but their insomnia continues. The aim of the present study was to investigate the changes in sleep parameters after abrupt discontinuation of zolpidem during an adequate therapy of insomnia.

Discontinuation of hypnotics during cognitive behavioural therapy for insomnia
BACKGROUND In practical sleep medicine, therapists face the question of whether or not to discontinue the ongoing use of hypnotics in patients, as well as the possible effects of discontinuation. The aim of this study was to evaluate the effects of discontinuing third-generation hypnotics on the results of cognitive-behavioural therapy (CBT) for primary insomnia in patients after long-term abus...

PURPOSE To determine the proportion of patients receiving hypnotics upon admission to a palliative care unit, the frequency and intensity of withdrawal symptoms after rapid hypnotic discontinuation, and the effect of discontinuation on insomnia and cognitive failure. PATIENTS AND METHODS 120 consecutive admitted patients. Rapid hypnotic discontinuation (1-4 days) was attempted. Insomnia (visu...
متن کاملDiscontinuation of benzodiazepines among older insomniac adults treated with cognitive-behavioural therapy combined with gradual tapering: a randomized trial.
BACKGROUND Long-term use of hypnotics is not recommended because of risks of dependency and adverse effects on health. The usual clinical management of benzodiazepine dependency is gradual tapering, but when used alone this method is not highly effective in achieving long-term discontinuation. We compared the efficacy of tapering plus cognitive-behavioural therapy for insomnia with tapering alo...
متن کاملLong-term outcome after discontinuation of benzodiazepines for insomnia: a survival analysis of relapse.
Discontinuation of benzodiazepine (BZD) treatment for insomnia can be a difficult task. Cognitive-behavior therapy (CBT) for insomnia, combined with a supervised medication taper, can facilitate withdrawal but there is limited evidence on long-term outcome after discontinuation. The objective of this study was to examine medication-free survival time and predictors of relapse (i.e., resumed BZD...
متن کاملClinical evaluation of zaleplon in the treatment of insomnia
Zaleplon is a pyrazolopyrimidine hypnotic used for the treatment of insomnia. Zaleplon binds preferentially at the α1β2γ2 subunit of gamma aminobutyric acid type A (GABAA) receptors in the central nervous system, and has a half-life of about one hour. Efficacy studies show that zaleplon is a suitable hypnotic for sleep initiation purposes. However, because of its short half-life, zaleplon is le...
